Corneal Transplant

Dr. Gulani performs the full spectrum of corneal surgeries, including the ability to completely replace a damaged cornea. This is called a corneal transplant and Dr. Gulani has extensive experience in all different forms of corneal transplant surgery.
When To Consider A Corneal Transplant
The cornea is the clear front window of the eye. It is transparent so that light can enter the eye unobstructed and carry image information to the retina, the “camera film” at the back of the eye. When the cornea is damaged, it may swell or may develop scarring, changing the image sent to the retina, and vision becomes distorted. In the progressive condition known as Keratoconus, the cornea gradually gets thinner, becomes more conical in shape, bulges forward, impairs vision, and may eventually require a corneal transplant.
Dr. Gulani can perform a corneal transplant, providing a new cornea to restore vision. This can be done as a single surgery, or in conjunction with other eye surgeries that reconstruct the front part of the eye (for example, the sclera (white part) or iris (colored part).
He has designed specialized instruments and surgical techniques in this area of surgery and teaches related concepts to other eye surgeons worldwide.
Artificial Cornea Transplant
For those who have severely impaired vision and are either not good candidates for a human corneal transplant, or previously have had unsuccessful corneal transplant surgery, which needs correcting, Dr. Gulani offers a new procedure to remove the damaged cornea and transplant an artificial cornea. In fact, he was the first eye surgeon in the entire state of Florida to offer this surgery, and one of the first in the world to be selected to perform it.
With the advent of the artificial cornea, Dr. Gulani believes creating an artificial eye is not a far-fetched dream.
Lamellar Corneal Transplant
The cornea is made up of very thin layers (hence, lamellar), and in some cases where the cornea is only partially damaged, Dr. Gulani can perform a partial thickness transplant. Depending on exactly where the corneal problem is, he can customize the transplant to a specific depth and extent, to repair the individual corneal defect and restore vision.
Keyhole Transplant For Fuch’s Dystrophy
Fuch’s Dystrophy is a progressive eye disease, which usually occurs after the age of about 50. The eye’s ability to drain water from the cornea becomes impaired, so that the cornea swells and may develop blisters. Vision becomes blurry. In the early stage, the blurriness is apparent when the person wakes, and gradually clears as the day goes on. However, as the disease progresses, blurriness becomes evident at all times. Over time, Fuchs' Dystrophy patients can become functionally blind, and at this point, a corneal transplant is generally recommended.
Dr. Gulani performs a new procedure for a posterior transplant. He has named it a Keyhole Transplant, (DSAEK/DLEK surgery) because he can achieve it through a very small incision and place the transplant without stitches. It provides new corneal tissue at the back of the cornea, where endothelial cells have atrophied.
Dr. Gulani has designed new instruments for successfully performing these transplants. To share the procedures with other eye surgeons, he has taught his surgical techniques at national meetings, and has given many interviews to the news media and to medical journals.
It is also important to note that all cornea transplant surgeries can be further enhanced through Dr. Gulani’s extensive experience with laser vision surgery.
